近年、革新的な医薬品の多くは新興のバイオテック企業から生まれています。一方で、そうした企業が日本やアジアでの開発に着手できないまま、治療の選択肢が届かない状況が続いてきました。私たちはこの「ドラッグ・ロス」を、政策の課題としてではなく、事業として解きにいきます。

On May 28, 2024, Oceanusbio, Inc. was established with the purpose of bringing innovative medicines created in the US and EU to patients in Japan and Asia.

Why We Started

Today, many of the most innovative medicines originate at emerging biotech companies. Yet development in Japan and Asia often never begins, and treatment options never reach the patients who need them. We are approaching this “drug loss” not as a policy debate, but as a business to be built.

What Comes Next

We will stay small and agile while operating across R&D, Regulatory Affairs, Medical Affairs, Commercial, Business Development, and Enabling Functions. By taking responsibility for the full path — from in-licensing through submission to launch — we aim to shorten the distance to the patients who are waiting.

欧米では使える薬が、日本では使えない。承認が遅れているのではなく、そもそも開発が始まっていない ― それが「ドラッグ・ロス」です。

「ドラッグ・ラグ」との違い

ドラッグ・ラグは、海外での承認から日本での承認までに生じる時間差を指します。時間はかかっても、いずれ届く。一方でドラッグ・ロスは、日本での開発計画そのものが存在しない状態を指します。待っていても届かないという点で、両者は質的に異なる問題です。

厚生労働省の整理によれば、欧米では承認されているものの国内未承認かつ開発に着手されていない医薬品は86品目（2023年3月時点）。うち特に開発の必要性が高いとされたものが14品目、次いで必要性が高いとされたものが41品目に分類されています。

影響が大きいのはどこか

とりわけ影響を受けやすいのが、患者数の限られる希少疾患領域と、小児領域です。想定される市場規模が小さいほど、企業が日本を開発計画に含める理由は弱くなります。しかし治療選択肢の乏しさという点では、まさにこうした領域こそ新しい薬の意味が大きい。必要性と経済合理性が逆を向いてしまうところに、この問題の難しさがあります。

これは科学の問題ではない

有効性や安全性が疑わしいから届かないのではありません。すでに欧米の規制当局の評価を経た医薬品が、日本では検討の入口にすら立っていない。つまり、届けるための意思決定と実務が設計されていないだけです。

裏を返せば、設計次第で変えられるということでもあります。私たちが事業としてこの問題に取り組んでいるのは、そこに解ける余地があると考えているからです。

A medicine available in the US and Europe is not available in Japan — not because approval is delayed, but because development never started. That is “drug loss.”

How It Differs from “Drug Lag”

Drug lag refers to the time gap between approval overseas and approval in Japan. It may take years, but the medicine eventually arrives. Drug loss refers to a state in which no development plan for Japan exists at all. Waiting does not help. The two are qualitatively different problems.

According to the MHLW’s own review, 86 medicines approved in the US or Europe were unapproved in Japan with no development even under way as of March 2023. Of these, 14 were classified as having a particularly high need for development, and a further 41 as having a high need.

Where the Impact Is Greatest

Rare disease and pediatric areas are affected most. The smaller the expected market, the weaker the reason for a company to include Japan in its development plan. Yet these are precisely the areas where treatment options are scarcest and a new medicine matters most. The difficulty of this problem lies in that inversion — where need and commercial logic point in opposite directions.

This Is Not a Scientific Problem

These medicines are not missing because their efficacy or safety is in doubt. Products already assessed by regulators in the US and EU simply never reach the starting line in Japan. What is missing is the decision-making and the operational work required to deliver them.

Which also means it can be changed by design. That is why we are addressing this as a business — because we believe there is room to solve it.

大手製薬から新興バイオテックへ

かつて革新的な医薬品の多くは、グローバルに事業を展開する大手製薬企業から生まれていました。こうした企業は日本法人を持ち、国際共同治験に日本を組み入れることが標準的な進め方でした。日本は、意識せずとも開発計画のなかに入っていたのです。

現在、新薬シーズの相当部分は新興のバイオテック企業が担っています。その多くは自国市場に集中しており、日本に拠点も、日本での開発経験も、規制当局との関係も持ちません。

「日本を入れない」は積極的な判断ではない

ここで見落とされがちなのは、日本が開発計画から外れるとき、それは日本市場を評価した結果であることが少ないという点です。多くの場合、そもそも検討の俎上に載っていません。限られた資金と人員のなかで、優先順位の外に置かれる ― 拒否ではなく、不在なのです。

だからこそパートナーが要る

必要なのは、日本・アジアでの開発と上市を引き受ける相手です。導入から承認申請、そして上市までを一貫して担えるパートナーが存在すれば、開発計画に日本を組み込む判断は、追加の負担ではなく現実的な選択肢になります。

私たちがこの役割を担いたいと考えているのは、構造が変わった以上、それを埋める機能もまた必要になるはずだからです。

There is no single cause. But the largest change has been in who creates new medicines.

From Large Pharma to Emerging Biotech

Historically, most innovative medicines came from large, globally operating pharmaceutical companies. These companies had Japanese subsidiaries, and including Japan in multi-regional clinical trials was standard practice. Japan was part of the development plan almost by default.

Today, a substantial share of new drug candidates originates at emerging biotech companies. Many are focused on their home market and have no presence in Japan, no development experience there, and no relationship with its regulators.

Leaving Japan Out Is Rarely a Deliberate Decision

What is often missed is that when Japan falls out of a development plan, it is seldom the result of evaluating the Japanese market. More often, it was never on the table. With limited capital and headcount, it simply falls outside the priorities — not rejected, but absent.

Which Is Why a Partner Is Needed

What is required is a counterpart willing to take on development and launch in Japan and Asia. When a partner can carry the full path — from in-licensing through submission to launch — including Japan becomes a realistic option rather than an added burden.

We want to serve that role because, once the structure has shifted, the function that fills the gap has to exist as well.

日本人第Ⅰ相試験の位置づけ

日本人を対象とした第Ⅰ相試験は、長らく日本参入の前提として扱われてきました。海外企業にとっては、これが日本を開発計画から外す実務上の理由の一つにもなっていました。

この前提は、すでに変わっています。2023年12月25日付の厚生労働省通知「国際共同治験開始前の日本人での第I相試験の実施について」（医薬薬審発1225第1号）により、国際共同治験に日本人が参加するにあたって、事前の日本人第Ⅰ相試験は原則不要となりました。日本参入の入口にあった最も大きな障壁のひとつが取り除かれたことになります。

迅速化のための仕組み

開発・審査を早めるための制度も整備されています。

• 先駆的医薬品の指定 ― 世界に先駆けて日本で開発される画期的な医薬品を対象とした優先的な取り扱い
• 条件付き早期承認制度 ― 患者数が少ない等の理由で検証的臨床試験の実施が困難な、重篤な疾患を対象とする仕組み。製造販売後に有効性・安全性を再確認する調査等の実施が承認条件となります
• 希少疾病用医薬品（オーファンドラッグ）の指定 ― 対象患者数の少ない領域における開発支援

適用の可否は個別に判断されますが、開発の初期段階でこれらを織り込んでおくかどうかで、その後の時間軸は大きく変わります。

早い段階で相談する

制度を活かせるかどうかは、規制当局とどの段階で認識をすり合わせるかに大きく左右されます。国際共同治験に日本を組み入れるかどうかの判断を含め、後戻りの効かない設計ほど、早く議論しておく価値があります。

Drug loss is now widely recognized, and the public response has been moving. The barriers to entering Japan are not what they were a few years ago.

The Role of Japanese Phase I Studies

Phase I studies in Japanese subjects were long treated as a prerequisite for entering Japan, and for overseas companies this became one practical reason to leave Japan out of a development plan.

That premise has already changed. Under the MHLW notification of December 25, 2023 (PSEHB/PED Notification No. 1225-1), a preceding Phase I study in Japanese subjects is in principle no longer required before Japanese participants join a multi-regional clinical trial. One of the largest barriers at the entrance to the Japanese market has been removed.

Mechanisms for Acceleration

Several frameworks exist to speed up development and review:

• Sakigake designation — priority handling for breakthrough medicines developed in Japan ahead of the rest of the world
• Conditional early approval — for serious diseases where a confirmatory trial is difficult to conduct, for example because the patient population is small. Post-marketing studies to reconfirm efficacy and safety are attached as a condition of approval
• Orphan drug designation — development support in areas with small patient populations

Eligibility is determined case by case, but whether these are built into the plan early has a decisive effect on the timeline that follows.

Engage Early

Whether these frameworks can be used depends heavily on when alignment with regulators begins. The less reversible the decision — including whether to include Japan in a multi-regional clinical trial — the more it is worth discussing early.

日本での上市は、承認を取った瞬間に始まるものではありません。その成否の多くは、開発のかなり早い段階の判断によってすでに決まっています。

1. 日本を国際共同治験に入れるか

どの程度の日本人データが求められるかは、疾患領域、既存の治療選択肢、そして開発の全体像によって変わります。一律の答えはありません。だからこそ、想定を置いたまま進めるのではなく、規制当局と早い段階で認識をすり合わせておくことが実務上の分かれ目になります。

3. 上市の実務を誰が担うか

承認は通過点であって、到達点ではありません。日本で医薬品を届けるには、製造販売業の許可、流通、薬価収載、安全性情報の管理、そして医療現場への情報提供が必要です。これらを誰が担うのかが決まっていなければ、承認が見えてから慌てて体制を組むことになります。

私たちは、この3つをまとめて引き受けられる相手でありたいと考えています。開発の入口で日本・アジアを「入れておく」判断ができるように、その先の実務を引き受ける。それが、ドラッグ・ロスに対する私たちの答えです。

Launching in Japan does not begin the moment approval is granted. Much of the outcome is already determined by decisions made far earlier in development.

1. Whether to Include Japan in the Multi-Regional Trial

This is the least reversible decision. Including Japan in a multi-regional clinical trial (MRCT) can minimize the additional Japan-specific studies required later. Adding Japan after the program is already running typically means a separate trial plan, with significant consequences for both time and cost. If there is any chance Japan will be considered, deciding early is better.

2. How to Secure Japanese Data

How much Japanese data is required depends on the disease area, the existing treatment options, and the shape of the overall program. There is no uniform answer. That is precisely why aligning with regulators early — rather than proceeding on assumptions — tends to be the practical dividing line.

3. Who Will Handle the Work of Launching

Approval is a milestone, not a destination. Delivering a medicine in Japan requires a marketing authorization holder license, distribution, reimbursement listing, safety information management, and medical information for healthcare professionals. If it is not clear who will carry these, the organization ends up being assembled in haste once approval comes into view.

We aim to be the counterpart who can take on all three. So that including Japan and Asia at the outset becomes a decision a partner can actually make — because someone is there to do the work that follows. That is our answer to drug loss.
